Delivery: To address specified by lessee (customer).  The lessee grants the right for Air

Filled Fun, Inc. to enter said property for the delivery and pick-up of equipment at approximately specified times.

I understand and acknowledge that the activity to be engaged with the rental of an inflatable unit(s) interactive amusement device, bring with it both known and unanticipated risks to myself and my guests.  Those risks include but are not limited to falling, slipping, crashing, and colliding and could result in injury, illness, emotional distress, death, or property damage to myself and my guests and invitee.

I do voluntarily release, indemnify, hold harmlessly and discharge Air Filled Fun from any and all liability, claims, demands, actions or rights of actions whether personal to me or to a third party which are related to arise out of or in any way connected with me renting an inflatable unit(s) those allegedly attributable to neglect acts or omissions.  I agree to reimburse any reasonable attorney fees and costs which may be incurred by Air Filled Fun in the defense of any such liability claim demand, action or right of action.  In the event that I file a lawsuit against Air Filled Fun, I agree that the substantive law of the state may apply in the action without regard to the conflict of the law of the state.

I acknowledge that I have adequate homeowners’ insurance, tenants insurance, or other liability insurance to cover any bodily injury or property damage which may occur to me, my guests or my invites for the use of the unit(s) I am renting.

Rules:   The lessee agrees to supervise both the equipment and its use at all times said the equipment is in the possession of the lessee.  Accompanying the contract is a set of directions for use and Safety Rules which I agree to follow and utilize at all times during the operation and use of the inflatable unit(s).

I acknowledge and certify that I have had sufficient opportunity to read this entire document, which I understand its content and I execute it freely, intelligently and without duress of any kind and agree to be bound by its items.

WAIVER

Although we strive to make all activities as safe as possible, activities of this nature do come with some elements of risks.  This waiver ensures that we have contact information on you and/or your children/guests and you are aware of these risks.

A complete waiver is required to participate in all inflatable activities.  We will keep on file, contact information for your child/guests for other activities at your request.

In consideration of being allowed to participate in any parties or events with Air Filled Fun the undersigned acknowledges, appreciates and agrees.

1. I knowingly and freely assume all the risks, both known and unknown, even if arising from negligence of the releases of others and assume responsibility for all participants.
2. I willingly agree to comply with the stated and customary terms, rules and conditions for participation.  If however, I observe any significant hazards during my participation, I will bring it to the attention of Air Filled Fun.
3. The risk of injury from this equipment can be significant, including the potential for paralysis and even death and while particular rules, equipment and safety discipline reduce the risk, the risk does exist.
4. I, for myself and behalf of my heirs, assign, personal representative and next of kin, hereby hold harmless Air Filled Fun and their officers, agents, employees other participants, and sponsoring agencies with respect to any or all injury, disability, death, or loss of damage to personal or property to the fullest extent of the law. 
5. By signing below for my children, guests and/or spouse, I agree to the above conditions when deciding to participate.

Safety Rules & Rental Agreement

Specific rules to follow:

1. Compatible age groups must play on the inflatable unit at the same time.  Age groups must NOT be mixed.  Different size children must NOT be mixed.
2. The jump house is designed for children and is not recommended for adults.
3. The maximum number of children in the inflatable jump house:

                              Ages 1 to 5 years old, 8 children

                              Ages 6 to 11 years old, 6 children

                              Ages 12 to 17 years old, 4 children

1. All persons must remove shoes, glasses, jewelry, belts, sharp objects, and anything else that might harm the inflatable unit.
2. No flips or somersaults.
3. All persons on water slides must wear a swim suit and slide feet first.
4. All persons on the water slides must NOT climb up or on the slide in an inappropriate manner, everyone MUST use the ladder.
5. No food or drinks inside or near the inflatable unit.  (within 15 feet)
6. Do not cut plastic ties or anything on the inflatable unit.
7. Do not tape or fasten anything to the inflatable unit.
8. Children/guests must be supervised at ALL times.  Please read the list of rules to ALL guests using the inflatable unit.
9. When unit is used with water, it is the responsibility to make sure that the water is turned off , the fan on and that the unit is still inflated when the Air Filled Fun representative arrives after the event.  ( for at least 1 hour) 
10. If rules are not followed when using water, customer is subject to additional fee.

Should the inflatable unit begin to deflate?

1. Remove all persons from the inflatable unit.
2. The motor has stopped:   check the cord connection at the outlet and the blower.
3. The blower continues to run:   check the air intake on the side of the blower for any blockage.  Check the inflatable tube to the blower for snugness and pull the strap for tightening if necessary.

I agree to accept for use as is, the inflatable unit listed on my Rental Agreement to accept full responsibility for the care of all equipment while it is in my possession.  I agree to pay full retail value of any equipment rented under this agreement that I do not return.  I agree to follow all the rules stated on this agreement.
